Standardized Testing - Woodstock

Cordis Academy will host ITBS (Iowa Test of Basic Skills) for grades K-12.  Testing is open to all home schooled students.

Test dates: May 17, 18, 19, 2011 for grades K-12 from 9:00-12:00

                  -K-5, 1^st and 2^nd grade tests are typically completed in two days of testing. 

                  - Parents should be in the parking lot by 11:30 for pick-up as some tests finish early.

                  - Parents should send snacks and drinks with their children.

                  - Parents should send 3 #2 pencils along with the child.

                  - Results will be mailed to you within 60 days.

Complete the form below and attach a check payable to MAFA.  All orders must be received by March 24, 2011 to ensure timely delivery of testing materials.  Orders may be dropped off at the Masters or Cordis offices.

I affirm that all students listed above fit the following definition of a home school student.  “A student is considered to be home schooled if the majority of the child’s academic education is privately funded and provided at home rather than in a classroom setting.  Students whose education is home-based but provided by publicly funded school would not be considered to be home schooled for the purposes of this test.”
